For physicians and dentists

  • It’s an animal study. The paper is mouse and ferret work, not human.
  • Not a regrowth drug for adults yet. The current human programme (TRG035, Toregem BioPharma) targets congenital missing teeth, not adults who lost teeth to decay or gum disease.

30

Healthy men in Phase I, each missing at least one molar, given one intravenous dose. The endpoint was safety. No published result shows a tooth erupting in a person on this drug.

Population context, not a drug result

These three figures are not trial endpoints. They describe how common the problem is.

  • 1%

Tooth number is abnormal in about 1% of people.

  • 7%

Complete edentulism in adults aged 20 and older. Source: World Health Organization.

  • 23%

Expert summary

Mechanism

USAG-1 (SOSTDC1/ectodin/WISE) is a bifunctional BMP and Wnt antagonist; the paper reports that BMP-directed blockade, rather than Wnt blockade, is what increases tooth number.

Key preclinical findings (Murashima-Suginami 2021)

  • Genetic rescue. USAG-1 knockout rescued tooth formation in Msx1-null mice (all 3 surviving double-KO mice had normal maxillary M3) and in EDA1-null mice, where about 75% of the controls had mandibular molar hypodontia. Msx1 double-KO was rare (3 of 151 offspring, vs. 1/16 expected), and the cleft palate persisted.
  • Antibody panel. Five monoclonal antibodies (#12, #16, #37, #48, #57) fall into BMP-only, Wnt-only, or dual-blocking classes. Those blocking Wnt or both (#12, #16, #48) were linked to low birth and survival rates in the offspring.
  • Efficacy. BMP-selective #37 reversed molar hypodontia in EDA1-null mice at a high rate and in a dose-dependent way, after a single intraperitoneal dose (16 µg/g) given to pregnant dams at E13. #57 induced supernumerary teeth in wild-type mice. Exact rescue percentages are in Fig. 3B and are not stated in the text. I could not extract them, so check the figure before quoting numbers.
  • Ferret (diphyodont model). #37 produced a lingual maxillary incisor resembling a third dentition, but it required 5× the dose (80 vs 16 µg/g), three administrations and immunosuppression, and the root was short.

Limitations to emphasise

  • Phenotype selectivity. The authors state that it did not cure all agenesis types and that causative-gene mutations may serve as patient-selection biomarkers.
  • Safety signal. Non-selective neutralisation had a safety problem in mice, which is why the BMP-selective clone was chosen.
  • Conflict of interest. The study was funded by Toregem BioPharma, and the institutions hold a related patent.
  • Species. The authors themselves call for further non-rodent work (dogs, pigs, suncus).
  • Other species. Dentistry.co.uk reports that dog data also showed third-generation teeth. I could not verify this in a peer-reviewed paper.

Clinical programme (TRG035, humanised anti-USAG-1)

Phase I (Kyoto Univ. Hospital, from Oct 2024)

Registered as randomised, double-blind, placebo-controlled dose escalation; target n=30 healthy men aged 30–64 missing ≥1 molar; single IV dose; primary endpoint safety, secondary pharmacokinetics and anti-drug antibodies. The Japanese registry lists it as complete.

Phase I results

Toregem says safety was confirmed, but detailed results have not been made public.

Phase IIa

Open-label, non-comparative, multicentre dose-finding study in 24 children with severe congenital agenesis. In Aug 2026 the PMDA completed its review of the notification (which is not drug approval); IRB review is pending and no start date has been announced.

Funding

About US$5.3M raised in May 2026; cumulative funding above US$29M.

Interpretation and trends

  • Efficacy in humans: none. TRG035 has not been shown to regenerate teeth in humans.
  • Target population. Severe congenital hypodontia (six or more missing permanent teeth) is the next-stage focus, not periodontal or traumatic tooth loss. Extension to acquired loss is an aspiration only.
  • Timeline. The team's 2030 target for general use should be treated as provisional.
  • Epidemiology. Tooth number is abnormal in about 1% of people. Complete edentulism affects 7% of adults 20+ and 23% of those over 60 (WHO).
  • Alternatives, all earlier-stage. Stem-cell/bioengineered teeth (the Tufts pig-human hybrid study), scaffold-based organoid approaches (KCL), and enamel and dentine regeneration biomaterials.

What to watch for

Phase I safety data and the anti-drug antibody rate, the Phase IIa dosing and eruption outcomes, whether root development and occlusal function are acceptable, and whether efficacy holds across different agenesis genotypes.
